Moisture due to urine and/or fecal incontinence softens the perineal skin, which makes the skin more susceptible to breakdown.
POLICY:
Skin care will be provided to patients who are incontinent of urine and/or feces using the appropriate cleaning solutions, moisture barriers and containment systems.
Patients who are incontinent of urine and/or feces will also be placed on a toileting schedule, as appropriate.
To manage incontinence, the reasons for the incontinence should be assessed:
Infection present
Impaction
Antibiotic administration
Infrequent toileting
