In the interest of patient convenience, blood will be drawn in the facility on all preoperative patients by the nurses of the facility.
PROCEDURE:
Vacutainers will be used to draw blood specimens.
Blood specimens will be labeled, in the presence of the patient, with patient information, date, time and initials of the nurse who drew the blood.
All specimens will have a Clinical Laboratory requisition form that will be filled out completely and accurately.
All blood bank orders are to be ordered on Blood Bank requisition forms.
