Patients may not be denied access to their medical record. Patients have the right to review their medical record.
PROCEDURE:
When a patient requests to review his/her medical record, the psychiatrist must be notified.
A written request stating the purpose for review must be obtained from the patient.
The Case Manager/Therapist upon advice from the psychiatrist will make an appointment with the patient to review the record together. The Case Manager/Therapist will be present to answer questions.
Details of the occurrence will be noted in the medical record.
