When the first appointment is missed, with or without cancellation, a Reschedule Notification Exhibit A is sent to the patient.
If the missed appointment was for a follow-up on a medical condition and the patient has not rescheduled within one week, a certified letter should be sent requesting that the patient contact the office Exhibit B. After the second certified letter is sent to the patient Exhibit C and there is no response, form letter D is sent to the patient along with a copy that is sent to the patient's HMO.
