Dear Patient,
You have the right to refuse any medical or surgical treatment that you do not want. In addition, you have the right to formulate an Advance Directive. This means that you can write down your wishes concerning your health care and whether or not you would want to be given life-supporting treatments when there is no hope of recovery, as in a terminal illness.
This directive would not mean that less care would be given to you, but would only insure that your wishes would be followed by healthcare personnel, including your physicians and your family.
