Hospital shall provide patients with a smooth transition to different levels of care within the facility. To facilitate this process, all disciplines providing services for the patient will participate in the transfer process as appropriate to the scope of services within their department.
A physician's order shall be required prior to any intrafacility patient transfer.
Manner of transport either by wheelchair or stretcher, shall be determined by the physician ordered activity level and patient condition.
