To provide guidance for the assessment and reassessment of the psychosocial needs of patients, families or caregivers.
POLICY:
The Hospice social worker or chaplain may provide psychosocial services to the patient, the family or the caregiver to help them cope with and adjust to a patient's terminal illness, make final arrangements and assess bereavement needs.
PROCEDURE:
A psychosocial contact is made either by a home visit or by telephone within two 2 business days of the patient's admission. A home visit is the desired method to initially complete the psychosocial assessment.
