To specify the attending physician's role in the delivery of medical and Hospice services to patients.
POLICY:
The medical care of each patient admitted to Hospice is the responsibility of the patient's attending physician. Communication between the attending physician and other members of the interdisciplinary team will be ongoing and documented in the patient's medical record. When the attending physician is absent, the attending physician must designate another physician to oversee care for the patient.

The physician may make a referral when the patient, family or caregiver agrees to Hospice care. Hospice needs the following:
The diagnosis, recent history, physical exam and symptoms
Current medications and treatment
Certify that patient is terminally ill with six 6 months or less to live
Provide a DNR/DNI order
