To provide guidelines for the appropriate assessment of patients who may have functional limitations requiring assistance and services from Hospice.
POLICY:
During the initial assessment visit the functional status of the patient is assessed for the provision of appropriate services.
PROCEDURE:
During the initial assessment, the Hospice RN performing the admission visit will assess the following but not limited to:
Level of independence in the home environment for the following:
Eating
Toileting
Transfers
Walking
Shopping
Cleaning
Laundry
Bathing
Dressing

Once the level of functioning is determined, the RN will decide which additional Hospice interdisciplinary team services or further functional assessment the patient may benefit from, including home health aide, therapies, Physical, Occupational and Speech, medical social worker services and volunteers.
