Hospice staff, patients, patient representatives and attending physicians may request to attend meetings whenever discussion may be about the care involving an individual patient.
Hospice maintains minutes of these meetings. To assure confidentiality, any discussions involving individual patients or Hospice staff will not include names. Identification numbers will be used instead.
