To establish the process to follow when a patient dies at home while receiving care from Hospice.
POLICY:
Hospice anticipates that patients will die at home. When a patient dies the RN, Social Worker and/or the Chaplain may visit the residence. The family or caregiver may require skilled nursing intervention, psychosocial support and practical assistance immediately after the death.
PROCEDURE:
Following notification of a patient's suspected death, the RN or other Hospice interdisciplinary team members will help the family or caregiver. Visits will occur whenever necessary.
