To outline the process by which a patient or patient's legal representative may choose to receive Hospice care from another provider.
POLICY:
Patients receiving Hospice services under the Medicare/Medicaid Hospice benefit may change the designation to another Hospice once in each election period.
PROCEDURE:

The date the change is to be effective
