To ensure the safe administration of medications.
POLICY:
Medications will be administered from properly labeled containers only. No self-prescribed patient medications will be given by Hospice personnel.
PROCEDURE:
All prescriptions will be labeled by the pharmacist with the following:

Prescription number
Patient's name
Date of filling
Physician's name
Name and strength of medication
Directions for administration
If necessary, the Hospice Case Manager will obtain additional medication information.

| | ...No medications will be given from non-labeled containers. If there is any reason to suspect the drug found in a container is not the drug labeled, the medication will be held until its identity is verified.
Labels will be carefully checked for expiration date and the medication should be checked for stability, i.e., without indication of deterioration, damp, cloudy, discoloration.
