To ensure that patients have families and caregivers or adequate resources/plans to care for the patient in the home 24 hours a day, if the patient's condition warrants.
POLICY:
Hospice does not provide a primary caregiver 24 hours a day for the patient. Patients who do not have a primary family member or caregiver will develop a plan with the Hospice Social Worker to provide for care when their condition dictates the need for additional care.
