In the event that staff communicates with a client/family member over the telephone, the Telephone Discussion Record will be completed and placed as a permanent part of the client's medical record. Telephone discussions may be utilized in the following circumstances:
The staff contacts the client in the interim of home visits to assess his/her medical condition and/or compliance with instructions.

Client name as recorded in the agency medical record
Medical record number
Date of the telephone discussion
