To explain the conditions and process by which clients are discharged from Agency's care or service.
POLICY:
At the time that care or services are discontinued, the clients must meet the agency's criteria for discharge. A client will be discharged by representatives of agency if one or more of the following criteria apply:
If receiving Medicare reimbursement, skilled care is no longer required.
If receiving Medicare reimbursement, the client is no longer homebound.
The condition of the client is stable and/or proper arrangements for providing care/services are made.
