The purpose of the individualized client Pharmacy plan of care is to identify medication-related problems, to establish outcome goals and to identify pharmacy monitoring and interventions related to the therapy provided. Planning of care is performed to ensure that care and services are adjusted to the client's specific need and problems. The plan of care is reviewed and adjusted according to the client's current problems and clinical needs.
