Clients shall have a nutritional needs assessment completed at the time of admission.
PROCEDURE:
Height and weight will be assessed, as clinically indicated, for all home infusion clients by the pharmacist or nurse completing the initial assessment.
The Registered Dietitian will be responsible for further assessments as outlined below. If the home healthcare agency does not have a Registered Dietitian on staff, or one is not available, the will provide a Registered Dietitian to complete a more indepth assessment and intervention via telephone consultation.

Male 106 pounds for first 5 feet, plus 6 pounds for every inch over 5 feet
Female 100 pounds for first 5 feet, plus 5 pounds for every inch over 5 feet
New weights will be obtained at least every two 2 months for ongoing, home infusion clients as clinically indicated. Those who have unintentionally lost 10 or more pounds over the first two 2 months or less of service will be referred to the Registered Dietitian for further assessment.
