Infusion Therapy Services's criteria for transfers and discharges of clients will depend on the assessment of the client's needs. The discharge process will allow for continuity of care.
The client's needs will be communicated to the new providers as necessary, at the time of discharge.

Completion of prescribed therapy
Client admission to hospital or skilled nursing facility
