The Agency honors client rights as adopted by its Governing Body and informs the client of his/her rights and responsibilities in the care process before or at the time of admission.
The Treatment Consent, Assignment of Benefits, Agreement to Pay and Release of Information Agreement forms clearly state these rights and responsibilities. The client is given a copy upon admission, and the information from the forms is disbursed to agency staff.
