The organization does not knowingly or unwittingly submit claims:
For visits:
Not made;
To beneficiaries who are not homebound;
To beneficiaries not requiring a qualifying service;
Not authorized by a physician;
Not medically necessary, excessive or otherwise inappropriate even if ordered by a physician.
For equipment and/or supplies:
That were never provided or were not provided as claimed.
That were not medically reasonable and necessary given the patient's documented medical condition.
All claims submitted to Medicare, Medicaid and other third party payors are supported... |
