Hospital shall provide a patient with protected health information upon request.

Any requests for protected health information by a patient are to be made in writing.
The patient may request that the protected health information be sent to an alternative address if the patient can prove that the healthcare information could cause him/her harm. An alternative address is one that the patient feels is secure and that the information will not be inadvertently placed in someone's possession who could potentially cause the patient harm by having access to the information.
